It is understood that the above named organization and contact person will adhere to all applicable facility and/or equipment rental policies and procedures. Furthermore, the named organization and contact person will be responsible for personal liabilities and property damages to the facility/equipment. A copy of Certificate of Liability Insurance is required. The Certificate of Liability Insurance must be submitted no later than 10 business days prior to the actual event. Certificates can be uploaded with this request or emailed to
